Analysis of K3 Implementation with FMEA and FTA Methods at PT. Sumber Alam Santoso Pratama Banyuwangi

In today's industrial business world, Occupational Safety and Health (K3) is indispensable. But in reality there are many things that hinder the production process due to work accidents. Because in a production company has a target work time that must be completed quickly. One of them work accidents occurred at PT Sumber Alam Santoso Pratama. The purpose of this study is to analyze the causes of work accidents and determine the level of work productivity. The methods used are FMEA and FTA. Research results based on FMEA method is the calculation of the highest RPN is in the activity exposed to the machine with a value of 37.5. While the results of the FTA method are the level of productivity within 3 years, namely 0.998, 0.997 and 0.996. After that, the analysis using the failure factor tree obtained the results that there was negligence from employees towards the importance of using personal protective equipment (PPE) and factory management that did not provide socialization about the importance of work safety.
